All clients must fill out our intake form and submit required documentation before making an appointment. This is to help our counselors review your case thoroughly. If you need assistance with the form please call our office to set up an intake orientation. If you do not need assistance there are two ways you can fill out the form and submit your documents.
1. Press the button "Intake Form" below to link you to our online intake form. When linked to the intake choose either "Homeowner experiencing a financial hardship" as a service type.
(This method is recommended because it goes directly to our intake staff for review.)
If your hardship began on or after March 1, 2020, due to COVID-19 choose the option "Homeowner experiencing a financial hardship since March 1, 2020, due to COVID-19"
2. Download the intake form fill it out and to turn it in you can either fax, email or drop off the application along with all documentation directly at our office.
Due to COVID-19 restrictions we are only accepting documentation by appointment. We will not be able to accept your documentation without one.
